When making your application, it is essential that you complete the form that is attached below. It is imperative that it is completed fully. Please return the completed form to school and your child's name will be added to our waiting list.

Please note that if your child is baptised Catholic, you must attach a copy of their baptismal certificate in order for the correct priority to be given to your application. This is a very important aspect of the application, especially if school is oversubscribed.
